Acne Scar Treatment Session Breakdown (Part 2/2)

In the world of acne scar treatments, no two cases are the same. Some scars are soft and shallow, while others—like the ones we tackled today—are thick, deeply tethered, and decades old. These are the scars that hold people back the most, and they’re exactly the type I focus on day in and day out at Qazi Cosmetic Center in Orange County.

Step 1: Untethering the Scars with Subcision

After prepping the skin, I began the subcision process. This is where I break apart the fibrotic bands that are pulling down on the skin from below the surface. These bands are what cause that “dimpled” or “tethered” appearance that never quite goes away on its own.

As I worked through the tissue, we immediately saw and felt and heard the release.


Step 2: The Nokor Needle – For When It’s Really Stuck

Standard subcision needles are great for many cases, but for old, stubborn scars with really dense scar tissue, I bring out the Nokor needle.

This tool allows me to carefully and effectively release scar bands that are too thick for traditional subcision. It’s not about being aggressive—it’s about being precise and thoughtful. I move slowly, methodically, and with intention.

That instant improvement is what patients often describe as a huge emotional release, not just a physical one.


Step 3: Erbium Laser for Skin Resurfacing

After subcision, we want to refine the skin’s texture. The best way to do that—especially right after untethering—is with the Erbium laser.

This gentle resurfacing laser works on the upper layers of the skin to soften harsh edges, smooth out uneven tone, and improve overall radiance. It pairs beautifully with subcision because once we release the scar bands, we can actually resurface from the outside-in.

This dual approach sets the stage for the next phase: volume replacement.


What’s Next for the Patient?

Now that the tethered scars are released and the skin is resurfaced, we can begin restoring lost volume with collagen-stimulating fillers like Bellafill or Sculptra. This part of the journey takes time, but the foundation we laid today makes it all possible.
