Welcome to Qazi Cosmetic Center’s Terms of Service for our exclusive programs, memberships, and benefits. These Terms govern your participation in our programs, purchases, and any other exclusive offerings provided by Qazi Cosmetic Center.
By accessing or using our website, booking an appointment, or making a purchase, you agree to be bound by these Terms and Conditions and our Privacy Policy, which is incorporated by reference. If you do not agree to these Terms or the Privacy Policy, please discontinue use of the site immediately.
These Terms apply to all online and in-office purchases, consultations, memberships, and promotional programs offered through our website or affiliated digital platforms. By using this site or participating in our services, you confirm that you are at least 18 years of age and legally capable of entering into a binding agreement. If you purchase or schedule services on behalf of another person, you represent that you have the authority to act on their behalf and ensure their compliance with these Terms.
By enrolling in or participating in any of our programs or services, you agree to comply with these Terms and any additional guidelines or rules we may provide. Please read these Terms carefully before participating. If you do not agree with any part of these Terms, you may not enroll in or use our programs.
Our programs are designed to enhance your experience and provide additional benefits to our valued patients. Participation confirms that you are at least 18 years old or have obtained appropriate parental or guardian consent. These Terms apply to all participants, including members, patients, and any individuals enrolled in or utilizing Qazi Cosmetic Center’s services.
Intellectual Property and Website Use: All content on this website—including text, images, videos, logos, and graphics—is the property of Qazi Cosmetic Center and may not be copied, reproduced, modified, or distributed without prior written consent.
Unauthorized use of this website, including scraping, framing, or automated data collection, is strictly prohibited.
You may not use our website for any unlawful purpose or to solicit the performance of illegal activity.
Booking General Guidelines:
The following and similar treatments are categorized as a Category 1 Purchase.

General Booking Guidelines
- A deposit is required to secure every appointment, and your appointment is not confirmed until the deposit has been received.
- Your full invoice must be paid before any service is performed; if a balance remains, your service will not proceed.
- You are responsible for ensuring you are medically fit and eligible for treatment before you book; the same cancellation fees apply if treatment cannot proceed due to health changes, lack of medical clearance, pregnancy, or changes in finances.
- Any request to cancel or reschedule must be sent in writing by email or text, and it is your responsibility to confirm that Qazi Cosmetic Center (QCC) has acknowledged the request in writing.
- No-shows and late cancellations incur fees as outlined for your service category.
- Prepaid services must be used within one year of purchase; any unused portion after one year will be forfeited.
- Prepaid services and deposits are not transferable to another person, and (unless stated otherwise in your category) they are not transferable between services.
- For your safety, you will be evaluated immediately prior to treatment, and services may be deferred or declined if your provider determines the treatment is not appropriate or beneficial.
- All services must be scheduled and completed within 12 months of purchase, consistent with standard package expiration terms. Unused value after this period is forfeited with no refund or credit.
- Complimentary and discounted services are subject to availability and may require advance booking.
- We reserve the right to limit the enrollment and appointment slots for services performed under the warranty, rewards, and promotional services.

Category 2 Bookings and Appointments:
- To schedule a Category 2 appointment, a non-refundable deposit of $1000 or 10% of invoice deposit (whichever greater) is required.
- Full payment of the invoice is due 14 days prior to the scheduled date. If the balance isn't paid in full by 5pm PT on the 14th day prior to your appointment, the procedure will be canceled at 5pm PT on that due date. Cancellation fees will apply.
- A cancellation is defined as any request you make to terminate or postpone the scheduled procedure appointment date. Any request that you make to reschedule the appointment date will be treated as a cancellation. All cancellation requests must be communicated in writing via email or text message to Qazi Cosmetic Center (“QCC”), it is the patient’s responsibility to ensure that their cancellation request is received and acknowledged with a response in writing from QCC.
- If you cancel your appointment more than 21 days before the scheduled appointment date you will be allowed a transfer for the 10% of invoice total or $1000 deposit (whichever is greater) to other category 2 services performed at QCC.
- The procedure fee doesn't include lab tests, preoperative clearances, PCP visits, or medications, which are the patient's responsibility.
- For cancellations made within 14 to 7 days prior, a 50% invoice total or minimum of $1000 (whichever is greater) cancellation fee will be charged without refund or transfer options. Payments made beyond this amount may be refunded back to the original form of payment.
- Cancellations made within 7 days of the procedure result in a 100% of invoice total as cancellation fee and charges are non-refundable and non-transferable.
- Deposits and payments for Category 2 services may not be used towards a service after one year if the procedure isn't performed.
- Deposits may not be transferred to any other person and there is no option for refund.
Patients are responsible for ensuring they are fit and able for treatments prior to scheduling; the same cancellation policies apply for non-clearance due to health conditions, changes in finances, or pregnancy.

Category 3 Bookings and Appointments:

- You may not accumulate rewards points for services in this category.
- You may not redeem rewards points to pay for services in this category.
- Benefits earned from any membership tier may not be applied to pay for services in this category.
- Gift cards may not be used to pay for services in this category other than consultations, skincare products, or merchandise.
Cancellation Definition: A cancellation is defined as any request you make to terminate or postpone the scheduled procedure appointment date. Any request that you make to reschedule the appointment date will be treated as a cancellation. All cancellation requests must be communicated in writing via email or text message to Qazi Cosmetic Center (“QCC”), it is the patient’s responsibility to ensure that their cancellation request is received and acknowledged with a response in writing from QCC.
Cancellations, Tardiness, Refunds:
- A rescheduled appointment is defined as a cancellation and new booking.
- For missed appointments or changes to appointments for a category 1 treatments made less than 48 hours in advance, A fee of $150 or 10% of the invoice fee (whichever is greater) will be charged.A cancellation or rescheduling fee will apply for Category 1 treatments if the appointment is missed or a change is made less than 48 hours prior to the scheduled time. This fee will be the greater of $150 or 10% of the invoice total.
- Arriving more than 15 minutes late may require rescheduling or cancellation of the original appointment, incurring the same fees outlined above for cancellation within 48 hours period.
- For prepaid laser sessions, failure to arrive on time or change an appointment within the specified period will result in a cancellation fee of 10% of procedure price or $150, whichever is greater.
- For a Category 1 service, patients are required to inform the provider's office at least 48 hours in advance to cancel or reschedule an appointment.
- Refunds are available for Category 1 services canceled more than 48 hours in advance.
- Refunds are limited and are only considered when no services have been rendered and your request is made within the applicable cancellation window.
- Unopened non-prescription products may be returned within 7 days of purchase.
- Refunds are not available after services have been rendered.
- Category 3 treatments are non-refundable and non-transferable except for a consultation which has been cancelled with at least 48 hours notice prior to the appointment.
- Promotional Treatments are not eligible for refunds after purchasing. In the event a patient is deemed medically ineligible in writing from their provider for a treatment after purchase, the full amount will be credited to their account for future use for up to one year.
- Any refunds for cash will be returned with a check within 30 days of the refund request. You may pick up the check from our facility or pay for shipping fees for certified mail with signature.
Consultations:
- Consultations for any provider must be scheduled only after the payment has been made for the consultation.
- Consultations are never held or guaranteed without a consultation payment prior to your in person or telehealth visit.
- The $150 consultation fee may only be applied towards your treatment if scheduled and paid for on the day of your consultation visit.
- The consultation fee will be kept by QCC as a fee if patients fail to arrive at their appointment, arrive more than 15 minutes late, fail to complete their submissions in preparation for a virtual visit, or do not cancel or change their appointments within the outlined policies.
- Consultation fees cannot be applied towards merchandise or skincare products and are non-transferable.
- Once a consultation has been completed, you may no longer receive a refund for your consultation for any reason.
- Subsequent consultations require new deposit payments for each visit even if you are an existing patient.
Tiered Membership Terms & Conditions: At Qazi Cosmetic Center, we offer three membership tiers: Quartz, Crystal, and Sapphire, available to individuals aged 18 and over, and excludes businesses, clubs, and organizations. Memberships are non-transferable and non-refundable, and benefits are exclusive to the registered member. Any unauthorized sharing of benefits will result in immediate termination of membership without the possibility of refunds. All annual memberships will expire one year from purchase. As of November 20th, 2025, the Crystal and Sapphire membership programs are at capacity and enrollment is no longer available.
Membership Tiers: Below is an overview of our different membership tiers and their benefits per yearly cycle. Please read the entire section for additional information and important details regarding each benefit.

- Patients are enrolled in this Membership automatically after any purchase is made

Sapphire Membership:
- $1,600 non-refundable annual fee to enroll
- $300 off Birthday Rewards Program
- $100 off Referral Rewards Program
- Free unlimited Dysport and Xeomin units towards upper face treatments (Frown, Forehead, Crows Feet)
- 20% off all other Dysport and Xeomin treatments.
- One free filler treatment with one syringe of Hyaluronic Acid filler for either lips, marionette lines, nasolabial folds, chin or cheeks.
15% off packages of the following:
- Accure Acne Laser
- Pico Laser
- Vascular Laser
- Morpheus8
- Micropeel Laser Facial
- PRP
- Tattoo Removal
- Microblading Removal
- Microneedling
15% off the following individual treatments:
- Microgold Facials
- Skincare Products
- Injectable Filler
Obtaining Membership:
- Upon purchasing a service at Qazi Cosmetic Center, all patients are automatically enrolled in our membership program at the Quartz Tier. This tier grants access to basic membership benefits outlined in our program details.
- To access higher membership tiers of Crystal and Sapphire and unlock additional benefits, patients may purchase upgraded memberships by contacting our team directly or purchasing on our website. Crystal and Sapphire tiers offer enhanced perks and privileges tailored to meet the diverse needs of our patients.
Eligibility:
- Membership is available to individuals aged 18 and over.
- Businesses, clubs, and organizations are not eligible.
Non-Transferability:
- Memberships are non-transferable, and benefits can only be availed by the registered member.
- Unauthorized sharing of benefits will result in immediate termination of the membership without any refunds.
Service Limitations:
- Complimentary and discounted services are subject to availability and may require advance booking.
- We reserve the right to limit the appointment slots for discounted and promotional services.
Pricing and Services:
- Qazi Cosmetic Center reserves the right to modify or discontinue services, treatments, or prices without notice.
- Members may be given alternative options at similar value to change services if not available.
- Discounts provided under all membership tiers are applicable solely for package pricing.
- Certain providers may apply a 15% surcharge for procedures scheduled on weekends.
- Quotes and proposals will be valid for 30 days from the creation date. If the price of your quoted treatment changes, you will be charged the quoted amount if purchased within those 30 days. If you plan to purchase your quote and proceed with treatment after 30 days, current prices will be applied to your invoice.
- All treatments purchased must be serviced within one year of purchase.
- Prices may vary depending on the provider and the severity or complexity of the procedure.
Membership Duration: All yearly memberships will cancel one year after the purchase date.
Cancellation & Refund of Membership:
- Membership fees are non-refundable.
- In the event of a cancellation during the membership year, members shall retain access to benefits until the conclusion of their current membership term unless they have been released from the membership program or practice.
This program provides participants with a 5-Year Loyalty Discount on eligible subsequent treatments. It does not imply complimentary or free services. All services qualifying under this program will be chargeable at a 25% discounted rate off the current list price at the time of booking.
- Abuse of the membership, including failure to follow policies, frequent last-minute cancellations, or any inappropriate behavior, may result in termination of membership without refund.
- Benefits and rewards have no cash value.
- QCC may remove any patient from their membership program at their discretion without refund.
Provider Discretion & Changes:
- All treatments and services offered under this membership or rewards are provided at the sole discretion of the attending provider.
- Your designated provider may change over the duration of your membership, but all our providers are credentialed in accordance with legal requirements.
- Certain providers may apply a 15% surcharge for procedures scheduled on weekends.
Online Orders and Shipping: Products purchased online are shipped within the United States unless otherwise stated. Estimated shipping times are provided for convenience but are not guaranteed. Qazi Cosmetic Center (“QCC”) is not responsible for lost packages, shipping delays, or delivery issues caused by carriers, customs, theft, weather, or other circumstances beyond our control.
All products are packaged with care to ensure safe transit; however, QCC cannot assume liability for damage that occurs during shipping. Customers may select from multiple shipping options at checkout. For enhanced security, QCC strongly recommends selecting certified mail with signature confirmation to reduce the risk of theft or misdelivery.
All skincare, cosmetic, and physical product sales are final once opened or if any seals or tags are removed. Only unopened, non-prescription items may be returned within seven (7) days of delivery, in accordance with our Refund Policy. Customers are responsible for all return shipping costs.
You are responsible for all government taxes, fees, tariffs, import duties, postal/courier fees, brokerage or handling charges assessed on delivery or after delivery. QCC does not collect these fees from you and they may vary in cost.
Shipping rates vary based on destination. Orders totaling $99 or more before tax may qualify for free standard shipping within the United States.
Website Security: We use encryption and secure payment processing for online transactions. However, no system is completely secure, and we cannot guarantee absolute protection against unauthorized access.
By using our website, you agree not to attempt to breach or circumvent our security features, introduce malware, or disrupt operations.
Data Protection: We value your privacy. All personal information will be kept confidential and will not be sold to any third parties.
Amendments: We reserve the right to amend, modify, or terminate these terms and conditions, and/or the membership program, at any time and in our sole discretion without notice.
QCC Warranty Program:
- Eligibility for this program requires patients to make a one-time purchase of their entire package for the following services:
- Deep tissue CO2
- Fractionated Resurfacing Laser
- TCA Cross
- Subcision
- Deep Subcision
- Combination Laser Treatment
- Bellafill
- Accure Laser package of 3
- Pico Laser minimum package of 6
- Hair PRP minimum package of 3
- Facetite package of 1
- The entirety of an Acne Scar Combination Treatment Package Quote (may include a variety of treatments).
- Split or segmented purchases will not qualify for the QCC warranty benefits.
- Warranty Discounts: The warranty benefit refers to the duration of the availability of the service at our practice.
- The limited warranty benefits are valid for up to 5 years from the date of your enrollment and your initial purchase of your qualifying purchase.
- The “Warranty” does not imply complimentary or free services in the future. All services qualifying under the warranty will be chargeable at a discounted rate.
- Choosing to move forward with partial treatments on your quoted plan, disqualifies you from receiving the warranty program benefits
- The QCC warranty program benefit applies to the duration of the continuous availability of the particular service at Qazi Cosmetic Center and up to a maximum of 5 years.
- We reserve the right to amend, modify, or terminate these terms and conditions, and/or the membership program, at any time and in our sole discretion.
- All treatments and services offered under this program are provided only at the sole discretion of the attending provider.
- Patients must be in compliance with the instructed pre-procedure care, home care or follow up care. It is at the attending provider’s discretion if the patient was not compliant with their instructions and if they are disqualified from the program.
- Your designated provider may change over the duration of your membership, but all our providers are credentialed in accordance with legal requirements.
- QCC has a right to disqualify a patient from the program at any time without any reason.
- We reserve the right to remove a patient from the warranty program with or without cause or based on documented non-compliance with provider instructions, abuse of the program's terms, or inappropriate behavior on premises, or as determined by the attending provider.
- You may use a gift card for discounted purchases made in the warranty.
Rewards Program Terms & Conditions:
- Members earn 1 point for every $1 spent on eligible purchases.
- Points are rewarded to the member's account upon transaction completion.
- Taxes, certain products, gift cards, and services are excluded from point collection.
- Points can be redeemed at a rate of 20 points for $1 towards eligible services, translating to a 5% rewards return.
- Points are non-transferable and can only be accrued and redeemed by the member who earned them.
- Unauthorized transfer, selling, purchasing, brokering, or bartering of points is prohibited, leading to forfeiture of the points.
- Points remain valid for 12 months from the earning date. Unused points will be forfeited post this period.
- Points have no cash value and are only redeemable towards eligible services, not for cash, products, gift cards, or taxes.
- Once used, credits and rewards cannot be refunded or reapplied. Once used, the credit or reward cannot be reused or returned to the patient profile.
- We may modify, amend, or terminate the rewards program and its structures at our discretion without prior notice.
- Misuse, fraud, or terms violation may lead to suspension, account termination, and forfeiture of points.
- International Customers: Open to both domestic and international clientele.
- Employees and Family are ineligible for point accumulation or redemption if purchases are made through employee or family discounts.
- Points are awarded only for services rendered to the account holder.
- Changes to the program will be announced on our official website.
- Adjustment Limitations: Post-transaction point adjustments are not allowed, barring system errors.
- There's no cap on how many rewards points a patient can accumulate on their account.
- Data from the rewards program is solely for program management. We uphold the confidentiality of this data and do not share or sell it externally.
- Category 3 services are excluded from earning rewards
- Category 3 services may not be paid using rewards.
- Points and rewards will be redacted immediately after a refund of fees has been made to an account holder.
- Rewards are not applicable for redemption on Qazi Academy services.
- Qazi Cosmetic Center reserves the right to change the eligibility terms at any time.
Patient Benefits Programs:
- These accumulated credits and rewards are non-transferable, cannot be redeemed for cash or cash equivalents, and are not applicable towards the purchase of physical products or gift cards.
- Existing patients (a patient who has paid an invoice) receive $100 credit for each new patient referred who purchases $500+ in services.
- The existing patient will receive the reward once the service has been completed on the referred patient.
- The referred patients also receive $100 credit toward their first purchase of services over $500.
- Each new patient may only claim one referrer.
- No limit to the number of referrals an existing patient may provide.
- Once the referred patient completes their service purchase, both parties receive referral rewards credits. Qualifying service purchases include treatments and services only, and do not include category 3 services.
- Birthday and Referral rewards credits are not applicable for redemption on Category 3 services.
- Once used, credits and rewards cannot be refunded or reapplied. Once used, the credit or reward cannot be reused or returned to the patient profile.
- Birthday Rewards must be used for services purchased in the birthday month. Birthday Rewards cannot be applied to any past or future invoices that fall outside of the birthday month.
- A minimum purchase of $500 in services is required to redeem membership Birthday Rewards.
- Credits and rewards from multiple benefits can be combined, provided the minimum purchase requirements for each benefit are met concurrently. The total invoice must equal or exceed the sum of the individual minimums for all applied benefits. If the combined minimums are not met, the credits cannot be used for that particular transaction.
- Referral rewards credits are valid for one year from the date of eligibility.
- There's no limit to the number of new patients an existing patient can refer to Qazi Cosmetic Center. However, a new patient can claim only one referrer for their credit.
- Rewards are not applicable for redemption on Qazi Academy services.
Subscription Terms and Conditions:
- Monthly billing occurs on your enrollment date.
- Each paid month includes one (1) treatment.
- Two-month minimum commitment required.
- Treatments must be used within the billing month and do not roll over.
- Unused treatments expire at the end of each billing month.
- Cancel anytime after month 2 through our portal or via email to stop future charges.
- No refunds for unused treatments.
- Cancellation will take effect after your current paid period.
- Treatments are non-transferable and can only be used by the subscription holder.
- All services are performed by licensed providers.
- Treatment scheduling is subject to availability.
- Subscription and treatments cannot be shared, sold, or transferred to another person.
- Treatment type must be selected from eligible subscription services.
- We reserve the right to modify these terms with notice to subscribers.
- You must be evaluated and diagnosed to make sure you are eligible for treatment before your service is performed. Purchasing a treatment does not guarantee eligibility.
Social Media Consent for Discount:
- Social Media Sharing Discounts may be offered during specific promotional periods to customers who provide explicit consent for us to film, photograph, or document their treatment experience and results for marketing purposes.
- Marketing materials may include but are not limited to social media posts, website content, advertisements, or promotional displays.
- If consent is revoked after the discount has been applied, the customer will be required to pay the full balance of the regular price active at the time of purchase for the treatment or service.
- You have the right to revoke your consent, however, revoking consent will apply to future publications only.
- Any previously discounted amount will be added to the remaining balance which will be due immediately in order to revoke the consent.
- Customers who participate in specials or promotions that require social media consent for posting purposes must attend an in-person follow-up consultation after their treatment. Failure to complete this requirement will result in the customer being charged the full price of the treatment, procedure, or product received as part of the promotion.
- No personally identifiable information will be shared without additional consent, aside from the audio and visual content provided for marketing purposes.
- We ensure the content adheres to applicable privacy laws and regulations.
- Discounts are non-transferable and cannot be combined with other promotions unless specified.
- This offer is only valid for selected treatments or services, as detailed during the consent process.
- By accepting the discount, customers acknowledge that their consent is tied to the discounted rate and understand the terms regarding revocation and payment adjustments.

Dispute Resolution and Governing Law: These Terms shall be governed by and construed under the laws of the State of California, without regard to its conflict of law principles. Any dispute arising from or relating to these Terms, purchases, or services provided by Qazi Cosmetic Center shall be resolved through binding arbitration in Orange County, California, in accordance with the rules of the American Arbitration Association (AAA). By using this website, you waive any right to participate in a class or representative action against Qazi Cosmetic Center. If you are a patient, arbitration will be governed by your signed patient agreement, which controls in case of any conflict with these Terms.
Promotional Sales, Gift Cards & Annual Sale Offers: The Annual Sale (“Promotion”) is offered exclusively from 12:00 AM Pacific Time on November 24, 2025, through 11:59 PM Pacific Time on December 8, 2025. All gift card purchases made during this window are subject to the following terms, which supplement and form part of these Terms of Service.
Eligibility: Participation in the Promotion requires the purchaser to be at least 18 years of age. By purchasing a promotional item or gift card during the Promotion Period, you confirm you meet this requirement and agree to all applicable terms herein. New and existing patients are eligible to purchase gift cards. The promotion is open globally to any customer except for businesses.
Purchase Requirements: All promotional purchases, including gift cards, must be paid in full at the time of purchase. Acceptable payment methods are limited to: cash, cashier’s check, Zelle, PayPal, credit card, debit card, and Cherry financing. No alternative financing or deferred payment arrangements are permitted. To pay with cash, cashier’s check, zelle, or cherry financing, please call us at 949-336-7293.
Gift Cards: Gift cards are issued electronically as e-gift cards with a unique redemption code linked to the purchaser’s account after payment clears. Gift cards are non-refundable, have no cash value, and cannot be resold, transferred, or reassigned after first use. A gift card may be assigned to another patient only before any redemption occurs; once used, it becomes permanently tied to that patient and cannot be shared or reallocated.
- All gift cards expire 12 months from the date of purchase. Any unused balance after this period is forfeited.
- Gift cards may only be applied toward Category 1 and Category 2 services. They cannot be used for any Category 3 purchases, except for consultations, skincare products, and merchandise.
Promotional Use Restrictions: All items and gift cards purchased during the Promotion Period may not be combined with any other offer, discount, membership benefit, rewards point redemption, referral rewards credit, birthday reward, social media discount, or third-party promotion, unless expressly stated in writing by Qazi Cosmetic Center. This includes all benefits under the Quartz, Crystal, or Sapphire membership tiers and any active rewards points.
Rewards Program Exclusions: Rewards points cannot be used to purchase promotional items or gift cards. No points will be earned on promotional purchases, gift card purchases, or services paid for using a gift card. This aligns with the general exclusion of gift cards and non-eligible services from point accrual under the Rewards Program.
Patient Conduct & Safety Policy: Qazi Cosmetic Center is committed to maintaining a safe, respectful, and healing environment for our staff, providers, and patients. By entering our facility or engaging with our staff, you agree to adhere to the following zero-tolerance code of conduct. Violation of these standards will result in immediate termination of the patient relationship, cancellation of all future services, and potential legal action.
- Weapons-Free Zone: Possession of firearms, knives, mace/pepper spray, or any other weapons is strictly prohibited on Qazi Cosmetic Center premises, regardless of any concealed carry permits or licenses.
- Prohibition of Harassment & Abuse: Patients must not engage in sexual harassment, unwanted advances, or inappropriate physical contact towards staff, providers, or other patients.
- Zero Tolerance for Threats & Violence: Making verbal or physical threats, intimidation, or aggressive behavior is strictly prohibited.
- Respectful Communication: The use of abusive, demeaning, profanity-laden, discriminatory, or loud language is not permitted.
- Safe Environment: Patients are expected to maintain a safe space and must not engage in behavior that disrupts operations or creates a hostile environment for staff or other patients.
- Theft & Property Damage: Theft, attempted theft, or intentional destruction of Qazi Cosmetic Center property, retail products, or equipment will result in immediate expulsion and will be reported to law enforcement.
Consequences of Misconduct: QCC reserves the right to immediately terminate services and ask any patient violating this policy to leave the premises. In such cases, the patient may be permanently banned from the practice, and any deposits for that day’s appointment may be forfeited as a cancellation fee.
Provider Discretion & Service Availability: All services—whether purchased online, at full price, or via promotion—are provided at the sole discretion of the attending provider.
- Refusal of Service: Qazi Cosmetic Center reserves the right to refuse any service deemed medically inappropriate, unsafe, or in violation of our Patient Conduct & Safety Policy.
- Evaluations: You will be evaluated and diagnosed before any treatment is performed on you. Purchasing a treatment does not guarantee eligibility.
- Provider Assignment: Your designated provider may change over time; however, all providers are fully credentialed in accordance with legal and clinical standards.
Non-Transferability & Personal Use: The Promotion and all associated gift cards are for personal, non-commercial use only. They may not be resold, bartered, used for group gifting beyond a single pre-use assignment, or transferred to third parties for resale or commercial benefit.
Governing Terms: These promotional terms are incorporated into and governed by the full Terms of Service of Qazi Cosmetic Center. In the event of any conflict between general Terms and these promotional terms, the more specific promotional terms shall control. Qazi Cosmetic Center reserves the right to modify, amend, or terminate the Promotion or related terms at any time in its sole discretion.
Entire Agreement: These Terms, together with our Privacy Policy and any signed patient consents, constitute the entire agreement between you and Qazi Cosmetic Center regarding your use of this website and purchase of our products or services.
If any provision is found unenforceable, the remaining terms shall remain in full force and effect. Qazi Cosmetic Center reserves the right to change these terms at any time.
