If you’ve ever wondered what really goes on during a professional acne scar treatment, you're not alone. The world of scar revision can feel like a black box, especially when you’re dealing with deep, stubborn scars like ice pick or boxcar types.
That’s why at Qazi Cosmetic Center in Orange County, we believe in full transparency. That includes showing you what our process actually looks like, from start to finish.
This article is Part 1 of a two-part guide that follows a real patient through a real treatment session with Dr. Qazi. You’ll get a look at every step, from the numbing shots to the advanced lasers used to resurface and rebuild scarred skin.

Step 1: Marking the Skin for Acne Scars
Before anything begins, Dr. Qazi takes time to evaluate the scar pattern on the patient’s face. Each mark serves a purpose: guiding the treatment plan and allowing precise targeting of the deepest scar points.
This is where the strategy gets personal. Every scar is different, and this first step ensures we’re not just “treating the face,” but treating your scars, exactly where they need it.
Step 2: Using TCA Cross to Boost Collagen & Rebuild Deep Scars
The TCA Cross technique involves applying a high-concentration chemical solution (TCA, or trichloroacetic acid) directly into the pits of ice pick scars. It sounds intense, but when done correctly, it's incredibly effective at triggering a local collagen response that slowly fills in deep scars.
What makes this step so valuable is that it targets the deepest part of the scar—something most treatments fail to do. And when combined with the next steps, it becomes part of a powerful, multi-layered strategy.
Step 3: Numbing Shots
Comfort is key. Before we dive into the laser portion, we always numb the treatment area to keep the patient at ease.
It’s quick and effective. Within moments, the face is ready for the next, more intense treatment.
Step 4: Using the Ablative CO2 Laser to Remove Thick Acne Scar Edges
This is where the transformation happens.
The ablative CO2 laser works by vaporizing the thickened edges around boxcar and rolling scars. This helps blend the transition between scar and healthy skin, making the entire surface smoother and more even.
This step is all about control. Dr. Qazi adjusts the laser to work just deep enough— breaking apart scar tissue while staying within the safety threshold for the patient’s skin type.
Why Ice Pick Scars Are So Difficult to Treat
Let’s talk about the elephant in the room: why are ice pick scars so stubborn?
- They’re narrow but deep— almost like little tunnels in the skin.
- Topical products and microneedling can’t reach them effectively.
- Most lasers spread their energy too broadly, missing the pinpoint precision required.
That’s why Dr. Qazi’s treatment protocol matters. By layering TCA Cross with ablative CO2 and, in some cases, filler or subcision, we create a strategy that attacks the scars from all angles.
