Terms and Conditions for Collagen Biostimulators Model Program

By participating in this program, you agree to the following terms and conditions. Please read them carefully before submitting your application.

Eligibility

  1. Applicants must be at least 30 years old.
  2. Only individuals residing in or near Newport Beach, Orange County, Los Angeles, and surrounding areas are eligible.

Program Details

  1. The program offers a free full-face treatment using collagen biostimulators such as Bellafill, Radiesse, or Sculptra.
  2. As part of this program, participants will have their treatment results featured on Qazi Cosmetic Center’s training guides, social media platforms, including but not limited to TikTok, Instagram, Facebook, and other marketing materials.
  3. Selected participants must undergo the treatment by April 1st, 2025.

Deposit Requirements

  1. A reimbursable deposit of $1,000 is required to secure participation.
  2. Timely attendance is required to all three follow-up visits to be elegible for a full deposit refund.
  3. Failure to attend follow-up appointments or comply with these Terms and Conditions will result in forfeiture of the deposit.

Consent for Use of Media

  1. By participating in this program, you grant Qazi Cosmetic Center permission to capture and use photos, videos, and other media of your treatment results for promotional purposes.
  2. This media may be shared across various social media platforms, promotional channels and marketing outlets.
  3. Participants will not receive additional compensation for the use of their media in promotional content.

Additional Conditions

  1. Participants are required to disclose any pre-existing medical conditions or allergies that may affect the treatment or its results.
  2. Participants are required to attend a one month, three month, and six month follow-up visit in person at Qazi Cosmetic Center. Appointments must be attended to within 10 days of these milestones.
  3. The program does not cover travel expenses to and from the treatment or follow-up appointments.
  4. Qazi Cosmetic Center reserves the right to modify or terminate the program at its discretion.

By submitting your application and participating in this program, you acknowledge that:

  • You have read and understood these Terms and Conditions.
  • You agree to comply with all the stated requirements.
