Volume Replacement

Subcision, Volume Replacement, Scars - April 24th 2025

Hi, everyone. I’m Dr. Qazi, a skin restoration specialist based in Newport Beach, California. My practice focuses on treating acne, scars, pigmentation issues, and facial balancing. Today, I want to share an incredible journey with you—a case that truly showcases the transformative power of patience, advanced techniques, and personalized care. I will talk about subcision, volume replacement and more.

Volume Replacement

A Life-Changing Transformation Over Two-and-a-Half Years

This particular patient came to me with severe, deep scars that made her feel aged and significantly impacted her confidence and self-esteem. She was determined to do whatever it took to achieve the best possible results. Together, we embarked on a multi-step process that spanned over two-and-a-half years. The outcome? A life-changing transformation that left her ecstatic.

We started by breaking up scar tissue through subcision, followed by volume replacement using bio-stimulatory fillers like Bellafill and Radiesse. Additionally, we performed laser resurfacing with both ablative CO2 lasers (for thicker scars) and erbium lasers (for finer refinements). It wasn’t just about one procedure—it was about layering treatments strategically over time to rebuild the skin’s structure and restore natural beauty.

The key takeaway here is that even for very severe cases, there are ways to improve outcomes if you’re patient and methodical. This isn’t a quick fix; it’s a journey requiring dedication from both the patient and the practitioner.

Why Volume Replacement Matters

One of the most impactful steps in this patient’s journey was volume replacement. Many people underestimate how much volume loss contributes to the appearance of scars, especially deeper ones. By injecting small amounts of bio-stimulatory products like Bellafill or Radiesse into targeted areas, we can stimulate collagen production and "fill" the depressions caused by scarring.

For example, this patient initially had laxity and thinning skin around her scars. Using diluted Radiesse helped improve elastin production, giving her skin a tauter, healthier appearance. Over time, these micro-doses integrate seamlessly with the tissue, creating long-lasting improvements without looking artificial.

Volume replacement isn’t limited to fillers either. For certain patients, fat transfer may be a viable option, particularly in areas requiring more substantial augmentation. However, I always prioritize safety and precision, ensuring that every step aligns with the individual’s unique needs.

Addressing Common Concerns About Acne Scars

Many of my patients come to me after years of struggling—not only with acne but also with feelings of guilt and shame. They often blame themselves, thinking they used the wrong products or picked at their skin too much. But the truth is, acne is largely driven by genetics and immune responses beyond anyone’s control.

When someone suffers from cystic acne, their body essentially goes haywire, destroying skin tissue and sometimes even underlying fat. That’s why addressing scars requires a comprehensive approach tailored to each person’s condition. There’s no one-size-fits-all solution, which is why I emphasize customized treatment plans.

Understanding the Psychological Impact of Acne and Scars

Acne and its resulting scars don’t just affect physical appearance—they deeply impact mental health and social interactions. Many patients express feelings of loneliness, anxiety, and depression due to their skin conditions. Some report avoiding social gatherings or feeling judged by others. These emotional scars often run as deep as the physical ones.

Through my work, I aim not only to restore skin but also to rebuild confidence and foster connections. Witnessing a patient’s transformation—both physically and emotionally—is incredibly rewarding. It reminds me why I chose this path: to help individuals reclaim their lives and embrace their authentic selves.

The Importance of Choosing the Right Practitioner

Selecting the right skin specialist is crucial for achieving safe and effective results. Unfortunately, many patients have shared stories of negative experiences with practitioners who lacked the necessary skill or used inappropriate techniques. Some have endured multiple ineffective treatments, wasted money on unnecessary products, or suffered complications from poorly administered procedures.

To avoid such pitfalls, I recommend doing thorough research before committing to any treatment. Look for practitioners with extensive experience, positive patient reviews, and a portfolio of successful cases. During consultations, ask detailed questions about their methods and expected outcomes. Remember, your skin deserves the utmost care and expertise.

Questions Answered

  • Question: What would you recommend for max dosage for glutathione?
Dr. Qazi: I don’t typically use glutathione due to concerns about potential liver toxicity. While short bursts might be fine, I avoid treatments that unnaturally lighten the skin beyond its natural tone. My goal is always to restore your authentic complexion safely.
  • Question: Can you explain what volume replacement is?
Dr. Qazi: Absolutely! Volume replacement involves injecting substances like hyaluronic acid, Bellafill, Radiesse, or Sculptra into areas affected by scars. These volume replacement products help rebuild lost volume, stimulate collagen production, and smooth out depressions. Fat transfers can also be used in specific cases.
  • Question: Is it safe to do these laser treatments in the summer or best to wait until winter?
Dr. Qazi: Summer is actually one of our busiest seasons! We protect patients with visors and high-quality sunscreens like Heliocare, an oral antioxidant supplement. With proper precautions, laser treatments are perfectly safe year-round.
  • Question: What are your thoughts on injector miso guns? Does it reduce the risk of vascular occlusion (VO)?
Dr. Qazi: Not really. Miso guns can apply too much pressure, increasing the risk of occlusions. Precision and technique matter far more than the device itself when avoiding complications.
  • Question: Can lasers help with the scarring from HS (hidradenitis suppurativa) areas?
Dr. Qazi: Yes, lasers can help, but combining them with volume replacement—like Bellafill—is often more effective. HS frequently causes significant fat destruction, volume replacement is crucial for optimal results.
  • Question: Why are there so many cases now of filler staying on for years and migrating? What causes this?
Dr. Qazi: Some fillers, especially hyaluronic acid-based ones placed superficially, can last much longer than advertised (up to 8–12 years). Migration occurs because they draw in water. Proper technique and product selection are essential to prevent these issues.
  • Question: What sunscreen do you recommend?
Dr. Qazi: I love Misoaesthetic’s tinted sunscreen with iron oxide for hyperpigmentation. Another great option is a Korean brand my wife uses—it’s lightweight, non-greasy, and perfect for sensitive skin prone to shine.
  • Question: Is Banish Micro Stamp good?
Dr. Qazi: It’s okay for mild scarring but won’t make a significant difference for deeper scars. Stamping devices are generally better than microneedling pens, which can worsen boxcar and icepick scars.
  • Question: Can just doing TCA cross work for deep icepick scars?
Dr. Qazi: It’s questionable. While repeated sessions can eventually yield results, combining TCA cross with volume replacement and laser treatments offers faster, more dramatic improvements.
  • Question: Does fat grafting deliver better results compared to Bellafill?
Dr. Qazi: It depends on the patient. Fat grafting works well for larger areas, while Bellafill excels in discrete spots needing precise correction. Sometimes, a combination of both yields the best outcome.
  • Question: Do you only work with acne scars, or can you help with dark scars on my legs?
Dr. Qazi: I treat all types of scars, including pigmented ones on the legs. Whether it’s acne-related or not, restoring your skin’s health and confidence is my mission.
  • Question: Can you explain Bellafill?
Dr. Qazi: Bellafill consists of 80% cow collagen and 20% PMMA. After the initial collagen dissolves within a month, the PMMA remains, prompting your body to produce new collagen around it. Slow, meticulous application ensures natural-looking results.
  • Question: How many rounds of subcision are needed to see significant improvement on moderate scarring?
Dr. Qazi: Most patients only need one session. However, complex cases might require additional rounds. Subcision breaks up fibrous bands beneath the skin, allowing it to heal smoother.
  • Question: Do people ever have bad reactions to Bellafill?
Dr. Qazi: Rarely. Patients with autoimmune conditions or allergies to bovine products should consider a skin test beforehand. If lumps occur, they’re easily smoothed with medications like 5FU combined with Kenalog.
