  • Question: Hi, what topical treatments can I buy which will reduce the appearance use of my acne scars? Mine are really colored and have hyperpigmentation.
    Dr. Qazi: For over-the-counter options, Eucerin's anti-pigment cream combined with a topical retinoid at night can work wonders. Apply the Eucerin cream during the day and use the retinoid in the evening. These products can help fade discoloration and improve overall skin texture.
  • Question: How to address hyperpigmentation after acne and trauma to the skin?
    Dr. Qazi: Post-inflammatory hyperpigmentation (PIH) often responds well to lightening creams, such as those containing azelaic acid or hydroquinone. Consistent use of sunblock is also vital, as UV exposure can worsen pigmentation. In stubborn cases, professional treatments like Pico lasers may be necessary.

  • Question: Can you tell me what is the most effective treatment for boxcar scars?
    Dr. Qazi: Boxcar scars respond well to a combination of approaches. Subcision helps release tethered scar tissue, while Bellafill can add volume beneath the scar. Additionally, TCA CROSS technique creates controlled injury to stimulate collagen production, effectively reducing scar depth.
  • Question: Is Bellafill temporary? Do you have to reinject every so often?
    Dr. Qazi: Bellafill contains PMMA microspheres that remain permanently in the skin, providing long-lasting results. However, as natural fat diminishes with age, occasional touch-ups might be necessary, typically every 5-10 years.
  • Question: What do you think about suctioning after subcision?
    Dr. Qazi: I don't recommend suctioning post-subcision, as it can cause excessive trauma and lead to premature scab formation. Gentle handling and proper aftercare yield better results without unnecessary complications.
  • Question: Is there a difference between punch excision for ice pick scars and excision for boxcar scars?
    Dr. Qazi: Yes, punch excisions target individual deep scars by completely removing them, whereas elliptical excisions are better suited for wider boxcar scars, allowing for more precise reshaping and closure.
  • Question: When should we start getting preventative Botox?
    Dr. Qazi: Begin preventative Botox as soon as you notice dynamic wrinkles forming. Early intervention can prevent deeper lines from developing, maintaining smoother skin longer.
  • Question: Can TCA make the scar worse?
    Dr. Qazi: Improper application of TCOA can exacerbate scarring, especially if done on oily skin. However, when performed correctly, it generally leads to temporary inflammation followed by scar contraction and improvement.
  • Question: Do you know why darker skin people experience more hyperpigmentation when getting microneedling?
    Dr. Qazi: Darker skin tones naturally produce more melanin, making them more prone to post-inflammatory hyperpigmentation. Sun exposure post-treatment can trigger this response, highlighting the importance of diligent sun protection.
  • Question: Do you offer any courses?
    Dr. Qazi: Currently, I provide training exclusively for physicians due to high demand and limited availability. Estheticians interested in corrective skincare might consider pursuing further medical credentials to qualify for future opportunities.

  • Question: Best for PIE or PIH and what is the best thing so topical that could fix it?
    Dr. Qazi: For Post-Inflammatory Erythema (PIE) and Post-Inflammatory Hyperpigmentation (PIH), 10% azelaic acid is highly effective. Available over the counter, it gently reduces redness and discoloration without harsh side effects.

While Bellafill has become my go-to choice due to its semi-permanent nature and excellent results, other options like Sculptra or Radiesse can also be effective depending on individual needs and preferences.
