Why Microneedling Isn’t Enough for Deep, Thick Acne Scars (And What Actually Works)

Microneedling is everywhere. Social media influencers, skincare blogs, and even some med spas push it as a miracle solution for acne scars. But here’s the truth: if you’re dealing with deep, thick acne scars like ice pick or boxcar types, microneedling probably won’t move the needle (pun intended).

That’s not to say microneedling doesn’t have value. It can be helpful for improving skin texture, tone, and fine lines. But for the majority of people with pitted or fibrotic acne scarring, especially in the cheeks or temples, microneedling often underdelivers. In my practice at Qazi Cosmetic Center, we rarely recommend it for that reason.

Why Doesn’t Microneedling Work for Deep Scars?

Let’s break it down. Microneedling works by creating controlled micro-injuries in the skin to trigger collagen production. The problem? Those needles only go so deep. Most microneedling pens max out around 1.5 to 2mm—and deep acne scars often go far beyond that

.Plus, many acne scars are tethered by fibrous bands underneath the skin—a microneedling needle isn’t cutting through that. That’s why patients often report seeing little to no change after multiple sessions.

Here are some common reasons why microneedling doesn't cut it for deep scars:

  • Not deep enough: Microneedling can't reach the base of thick or pitted scars.
  • Doesn’t break scar tissue: It won’t release fibrous tethers holding the scar down.
  • Slow, minimal change: It takes several sessions just to see subtle improvements.
  • Not customizable: You can’t tailor depth or technique per scar with precision.

So What Actually Works for Deep Acne Scars?

At Qazi Cosmetic Center, we take a completely different approach. Instead of relying on one-size-fits-all microneedling, we use advanced combination treatments designed to reach deeper layers and actively remodel scar tissue.

Here are a few tools we actually use for deep, thick acne scars:

1. Subcision

This technique uses a needle or cannula to manually break up the scar tethers underneath the skin. Once those bands are released, the skin lifts and looks smoother. It’s often combined with collagen stimulators.

2. TCA Cross

This is a high-strength chemical spot treatment for ice pick scars. It stimulates collagen from within the scar base, helping it rise over time.

3. Ablative CO2 and Erbium Lasers

These lasers resurface the skin and help smooth out scar edges. At Qazi Cosmetic Center, we use pinpoint techniques to target only the damaged areas while preserving healthy skin.

4. Bellafill or Sculptra Injections

These long-term fillers stimulate collagen and help restore volume under rolling or atrophic scars.
